Service Coordinator

Position Title:             Service Coordinator

Hours:                        Monday-Friday: 8:00am-4:00pm

Location:                    Koch Campus in Minnetonka

Purpose:

The purpose of this position is to plan, manage, and design individualized service plans for people choosing services from Opportunity Partners, and to provide direction to those people in conjunction with the total program objectives.

Your Day-to-Day:

  • Manage the provision of rehabilitation services to our people served so that they are able to successfully accomplish their goals
  • Develop an assessment and appraisal process for each person in the program and determine courses of action
  • Provide instruction to people served on how to develop functional skills and abilities so that they can accomplish their goals
  • Assist each person on your caseload with personal adjustments so that are better able to cope with their disability and communicate effectively in their life
  • Assure compliance with regulatory reporting requirements
  • Facilitate communication and gather information from all program services within Opportunity Partners

Required Qualifications:

  • Designated Coordinator status is required and must be met through one of the following
    • Bachelor’s degree in a field related to human services AND 1 year of full time work experience providing direct care to people with disabilities
    • Associate’s degree in a field related to human services AND 2 years of full time work experience providing direct care to people with disabilities
    • Diploma in a field related to human services from an accredited post-secondary institution AND 3 years of full time work experience providing direct care to people with disabilities
    • Minimum of 50 hours of education and training related to human services and disabilities AND 4 years of full time work experience providing direct care to people with disabilities
  • Valid driver’s license, motor vehicle insurance, and reliable vehicle
