<p>Carla Colvin, Semi-Independent Living Specialist at Opportunity Partners-Anoka, goes above and beyond to provide individualized care for the people she supports. Her hard work and compassion have earned her national recognition as the 2013 Direct Support Professional Recognition Award winner for the state of Minnesota. Carla is one of only 46 people nationwide to receive this honor from trade association ANCOR. Carla is a strong advocate for the people she supports and consistently creates positive change in the individuals she works with. Carla received the award at the ANCOR conference in Washington, D.C. on April 30.</p>
<p>Carla developed a passion for people with disabilities when as a child riding the school bus she saw other children treating kids with special needs as &#8220;different.&#8221; Years later, she left a job in the insurance industry to follow her passion to care for people and has now worked in the field for 15 years. Carla goes the extra mile to customize her care to meet the unique needs of each individual. Carla says: &#8220;If you take the time to get to know what motivates the people you serve, it makes for some pretty amazing accomplishments. I make it a point to teach the people I serve that a person&#8217;s disability is not their identity, that we are all intentionally flawed to make us unique and that everyone has value.&#8221;</p>

<p><strong>Essential Functions</strong></p>
<p>Processes all employee benefit elections (i.e. health, dental, vision, flexible spending accounts, health savings account, life insurance, accidental death and dismemberment, short-term disability, long-term disability and retirement). Duties may include, but are not limited to:</p>
<ul>
<li>Effectively manages communications and reports to benefit carriers while ensuring accuracy and compliance with plan requirements
<li>Acting as primary liaison to all Third Party Administrators (TPA) for basic transactional administration
<li>Referring higher level issues to the Human Resources Manager
<li>Under the overall guidance and direction of the Human Resources Manager, assumes primary responsibilities for implementing and coordinating all aspects of the annual benefits open enrollment process</ul>
<p>Responsible for day-to-day case management of all leaves of absences (short-term/long-term disability, FMLA, ADAAA, and extended leaves) and worker’s compensation claims. Duties include, but are not limited to:</p>
<ul>
<li>Coordinating prompt review of each case, filing the appropriate state and/or federally mandated reports/forms and maintaining effective communication between both the employee and manager on the best strategy for return-to-work actions
<li>Consulting with the Human Resources Manager to determine reasonable accommodations needed for an employee to perform the essential functions of the position
<li>Serving as a representative on the monthly Corporate Safety Committee, preparing updated reports on the YTD worker’s compensation claims, identifying trends and possible prevention strategies while coordinating efforts with the Wellness Coordinator
<li>Providing bi-weekly short-term disability pay reports to payroll
<li>Preparing annual OSHA reports, distributing to all locations and adhering to posting procedures</ul>
<p>Maintains ADP’s HR and Benefits Solution system. Duties may include, but are not limited to:</p>
<ul>
<li>Processing change forms directly impacting benefits for either enrollment or COBRA purposes
<li>Acting as the Administrator to Employee Self Service and promptly responds to employees inquires
<li>Running monthly reports to complete monthly benefit billings</ul>
<p>Conducts bi-weekly new hire orientation on ADP registration, benefits eligibility, benefit plan details and electronic enrollment. Additional duties include, but are not limited to:</p>
<ul>
<li>Following up with new employees to ensure a smooth transition into the organization
<li>Answering benefit related questions
<li>Scheduling one-on-one benefit meetings as needed</ul>
<p>Special Projects as requested and/or approved by Human Resources VP and HR Manager. Duties may include but not limited to:</p>
<ul>
<li>Conducting salary surveys for specific jobs
<li>Conducting benefit surveys from other companies</ul>
<p>Serves as a resource for all employees. Duties may include, but are not limited to:</p>
<ul>
<li>Familiarizing staff with benefits package and conducting one-on-one meetings as required
<li>Assuring proper completion of all benefit related forms
<li>Clarifying organizational policies and procedures</ul>

<p><strong>Qualifications:</strong></p>

Develops and implements methods of instruction so that services are provided in a safe, feasible, and financially stable manner while maintaining Dunwoody curriculum-based philosophy of delivery.  Duties may include but are not limited to:</p>

<li>Delivering quality individualized classroom/training instruction that follows a lesson plan designed to deliver certifiable skills training
<li>Adapting written skills training curriculum to customized lesson plans &#038; individualized instruction delivery methods

<p>Provides coverage for Learning Options Instructors so that there is consistency in daily curriculums.  Duties may include, but are not limited to:</p>

<li>Teaching classes when instructors are absent

<li>Joining an existing group to increase support
<li>Providing 1:1 instruction
<li>Following lists of prioritized coverage/activities on a daily basis

<p>Participates as a team member so that client services are facilitated and that regulatory and licensing requirements are met.  Duties may include but are not limited to:</p>

<li>Documenting client goals and objective progress on a daily basis

<p><strong>Essential Functions</strong>:</p>
<p>Provides   leadership   and   consultation   services   to   residential   nursing personnel and staff so that standard medical practices are followed, procedures are adhered to, and quality medical services are provided to residential clients. Duties may include, but are not limited to: </p>

<li>ensures compliance with regulatory agencies;
<li>ensures procedures relating to medical care are reviewed and satisfy operational needs;
<li>ensures client charts are complete and meet standards; d) provides LPN’s with clinical direction;

<li>oversees employee training programs;
<li>oversees the psychotropic medication monitoring program.</p>

<p>Develops and maintains a system of health records and reports so that there is a central source of health information available. Duties may include, but are not limited to: </p>

<p>Hires,  trains  and  develops  direct  reports  and  oversees the general staff procedures so that programs are adequately staffed with qualified personnel. Duties may include, but are not limited to: </p>

<li>conducting orientations, staff meetings;
<li>evaluating direct reports.</p>

<p>Assess actual/potential health care needs of assigned clients and houses through observation, interview of client and staff and review of client records so that the clients receive good health care. Duties may include, but are not limited to: </p>

<li>coordinating care between health care providers;
<li>developing plan of care for medical/diagnosis/health issues;
<li>completing all health reports;
<li>providing written and educational support;

<p>Trains and designs training programs for clients and staff in the health and safety areas of the agency so that good health and safety practices are reinforced.  Duties may include, but are not limited to:</p>
