<p>From polka dots and poodle skirts to cat-eye glasses and leather jackets, 350 guests and 65 volunteers rocked and rolled at Opportunity Partners’ Fabulous ‘50s Gala on April 27 at the Hotel Sofitel in Bloomington.</p>
<p>When the party was over, more than $100,000 had been raised to support people with disabilities living, learning and working more independently.</p>

<li>Interpreting the agency’s policies to employees</li>

<li>Providing regular acknowledgement and feedback for SET Instructors through regular one-to-one meetings, once a week for new staff and at least once a month for existing staff, and as needed based upon employee performance</li>
<li>Monitoring the progress of employee career/individual development plans</li>
<li>Meeting with instructors on a regular basis to provide resources and advice on how to work with clients, to overcome barriers to employment, and to solve work site challenges</li>

<p>Coordinates SET services with existing community employers, Opportunity Partners staff, and other necessary parties so that any service issues are minimized and opportunities for existing and new business growth are maximized.  Duties may include, but are not limited to:</p>

<li>Meeting face-to-face with employers at existing large community job sites at least once a month to review community employer satisfaction levels and anticipate and/or respond to employer needs and concerns</li>
<li>Checking in with employers at existing smaller community job sites at least once a month via telephone or email contact and meeting face-to-face with these employers at least once a quarter to review community employer satisfaction levels and anticipate and/or respond to employer needs and concerns</li>
<li>Increasing potential for growth at existing community job sites by analyzing all opportunities for expansion/additional services and, with assistance from OP Sales Team and Assistant Director / SETS, approaching community employers with ideas for expansion and/or additional services</li>
<li>Prospecting for new business opportunities by marketing exceptional quality of SET services to existing community employers and seeking out potential external referrals</li>

<p>Coordinates new SETS with community employers, Opportunity Partners staff, and other necessary parties so that “start up problems” are minimized.  Duties may include, but are not limited to:</p>

<li>Attending meetings in anticipation of new site start – ups and gathering information regarding business / departmental needs</li>
<li>Assisting to determine viability of new site start – ups, and overseeing activities of Assistant Manager / SETS as they develop potential Client candidate pool with collaboration between SET Department and Service Coordinators and possibly Production</li>
<li>Hiring and overseeing the training of new staff and supported employment teams</li>
<li>Maintaining active presence on-site at new SETS throughout training period to ensure seamless service delivery by anticipating and responding to any challenges, issues, or concerns</li>
<li>Contacting the employer for new site start ups at least: four times per month for first month; one time per month for five months following and; at least one time per quarter for the remainder of the first year</li>

<li>Provides QMRP support to customers, personnel, interdisciplinary team members and other professional support individuals so that customer outcomes are met and licensing standards are assured. Duties may include, but are not limited to: a) coordinating efforts to ensure customer outcomes and needs are met; b) establishing and maintaining positive and effective working relationships with interdisciplinary team members; and c)   maintaining operating standards and practices.</li>

<div>·         Facilitates meetings so that all Interdisciplinary team members are informed regarding the progress of the client, gathers feedback and information to revise plan to optimize client progress. Duties may include but are not limited to:</div>
<div>o   Documenting and filing conference results</div>
<div>o   Communicating and directing changes with other program staff and interdisciplinary team members</div>
<div>o   Updating client files and databases with any known changes no later than one week after being informed of a client status change</div>
<div>o   Completing Vocational/Educational Action Plans (VEAP) and filing completed forms</div>
<div>o   Overseeing the quality of service provisions for each client while meeting all regulations</div>
<div>o   Evaluating and measuring progress and relaying findings to Interdisciplinary Team according to regulatory requirements</div>

<div>·         Assures program continuity by completing ongoing procedures and operations within Opportunity Partners. Duties may include but are not limited to:</div>
<div>o   Accurately completing attendance records</div>
<div>o   Facilitating communication and gathering information and documentation from other internal program services within Opportunity Partners</div>

<div>o   Compiling, ensuring accuracy and submitting billing information to accounts receivable each month within allotted timelines</div>

<div>·         Assists the client concerning personal adjustment so that the individual will be better able to cope with his/her disability and demonstrate appropriate behavior in his/her life. Duties may include but are not limited to:</div>
<div>o   Developing behavior program plans designed to meet the individual needs of the client. Maintaining ongoing and direct communication with a client’s respective Interdisciplinary Team per regulations</div>
<div>o   Compiling and analyzing gathered information to develop and implement client specific behavior support programs</div>
<div>o   Assisting direct support professionals in ensuring program requirements are met</div>
<div>o   Maintaining data privacy, HIPAA regulations and utilizing professional counseling approaches</div>
<div>o   Supporting individuals on caseload by assisting with the identification of skills, habits and physical capacities needed for employment, developing advocacy and communications skills for individuals along with positive approaches to behavior support (VEAP assessment)</div>

<div>·         Participates in training and development activities so that new ideas and/or concepts can be implemented into the program and required training completed. Duties may include, but are not limited to:</div>
<div>o   Completing 40 hours of in-service training per year (20 hours after two years of employment) through reading, viewing videos/films, or attending in-house training</div>
<div>o   Attending workshops and/or appropriate conference as available</div>
<div>o   Completing Right To Know, Vulnerable Adult, and Blood borne Pathogen training on an annual basis</div>
<div>o   Completing First Aid and CPR training and renews prior to expiration dates</div>

<div><strong>Qualifications: </strong></div>
<div>·       Knowledge customarily acquired through a bachelor degree in a field related to social services, and one year work experience with clients with disabilities or related conditions, a two-year degree in a field related to social services, and two years of work experience with clients with disabilities, or five years of full time (2080 hours) experience working with clients with disabilities. Employees without a four year degree are required to become ACBIS certified within one year and three months of employment start date</div>

<p>Assists clients in all areas of daily living so that progress towards independence is reinforced.  Duties may include, but are not limited to:</p>
<ul>
<li>Providing any necessary assistance to clients to maintain a positive personal image such as grooming, hygiene, clothing coordination, and social skills </li>
<li>Teaching and documenting clients’ financial management, such as banking, paying bills, shopping, budgeting</li>
<li>Discussing and encouraging recreational and leisurely activities, as well as acceptable social interactions and community participation, along with conflict resolution skills</li>
<li>Teaching personal safety skills, such as home safety, emergency disaster procedures, street safety, bicycle safety, vulnerability issues, and using and carrying proper identification</li>
<li>Teaching clients skills in using metro transit options, when applicable</li>
<li>Supporting clients’ employment expectations and standards and discussing the importance of responsibility and employment obligations</li>
<li>Teaching and supporting clients’ personal medical and health care plans</li>
<li>Delegating responsibility or notifying client employer or day program of any medication/treatment plan changes and appointments at least one week in advance, when applicable</li>
<li>Scheduling and assisting clients with appointments</li>
<li>Coordinating nutritional and menu needs with nursing or registered dietician, when applicable</li>

<li>Facilitating, organizing, and communicating shared community and social opportunities with other clients residing in all of Opportunity Partners’ homes</li>

<p>Provides supervisory support to the Program Manager so that services are consistently provided and a positive image of Opportunity Partners prevails.  Duties may include, but are not limited to:</p>
<ul>
<li>Scheduling of personnel and posting at least two weeks, but optimally one month, in advance, ensuring all shifts have minimal coverage</li>
<li>Assisting Program Manager with employee performance appraisals, including maintaining adequate documentation and knowledge to support</li>
<li>Completing Verbal Performance Notification forms and submitting to Program Manager to document employee performance that does not comply with company standards</li>
<li>Complying and adhering to Opportunity Partners policies and procedures including, but not limited to, confidentiality practices, completing company accounting responsibilities, following expectations in the Petty Cash Policy, and completing required client financial reports </li>
<li>Communicating issues of concern that are impacting to the agency, residential programs, and/or clients to the appropriate individuals</li>
<li>Providing designated orientation and on-going training</li>
<li>Making routine purchases to maintain an adequate inventory of house/cluster supplies</li>
<li>Acting as a liaison with neighbors and responding to any neighborhood issues</li>
<li>Maintaining adequate knowledge of Adult Foster Care guidelines, MN Rule 245B standards, and company policies and procedures to assist Program Manager to ensure that services are in regulatory compliance</li>
<li>Attending departmental APM meetings and relaying necessary information to direct care staff</li>

<li>SILS-IHW: Providing services within service authorization limits to ensure accurate billing and documentation of service hours</li>

<p>Participates in the assessment and individual program planning process with each client, so that outcomes can be obtained.  Duties may include, but are not limited to:</p>

<li>Meeting with each client at least monthly to review program plan</li>

<li>Assisting in the training of existing staff on changes in client healthcare, programs, vocational issues, etc</li>

<p>Fulfills requirements of staff development so that clients receive the best possible services.  Duties may include, but are not limited to:</p>

<li><strong><em><span style="text-decoration: underline;">Evaluation:</span></em></strong> Evaluates participants in situational assessment settings as requested by referral source so that a rehabilitation plan can be developed that will take into account each participant&#8217;s interests, skills and limitations.  Designs evaluation plans for individual clients so that needs are identified and evaluation methods are planned.  Duties may include, but are not limited to:<strong></strong>

<li><strong><em><span style="text-decoration: underline;">Vocational Counseling:</span></em></strong> Provides vocational counseling to participants on case load so that each participant receives an individually developed plan.  Duties may include, but are not limited to:<strong></strong>

<li>Recommending appropriate services, including Rehab Services, Social Services, HC DD Division support, etc<strong></strong></li>

<li>Providing crisis intervention services as necessary<strong></strong></li>
<li>Teaching, counseling, and helping to direct the goals of participants in evaluation, Work Adjustment Training, and transition<strong></strong></li>

<p> <strong>Qualifications: </strong></p>
<ul>
<li>A four year degree in social services is required. A Master&#8217;s degree in Vocational Evaluation or Vocational Counseling is preferred </li>
<li>Communication skills, both written and oral</li>
<li>The ability to understand, effectively use, and train program staff in the use of the P.E.A.R.L. interactional style</li>
<li>Knowledge of developmental disabilities, MI, brain injury and related conditions</li>
<li>Organizational skills</li>
<li>Knowledge of testing and evaluation techniques, including standard and nontraditional (paper/pencil and situational).</li>
<li>Ability to work with a wide variety of people</li>
<li>Ability to teach job techniques and duties</li>
<li>Ability to drive and have access to a dependable car</li>
<li>Ability to work under pressure and react quickly in emergency situations</li>
<li>Ability to work in a wide variety of situations, especially packaging and assembly work</li>
<li>Basic computer skills</li>
