<p><strong>Essential Functions</strong></p>
<p>Provides on-the-job instruction so the client is able to meet the expectations of the employer, and works with the client to increase their skills and overcome potential barriers to independent placement. Duties may include, but are not limited to:</p>
<ul>
<li>Working with clients to build skills and self-confidence and providing constant feedback, striving for continued individual client progression
<li>Individualizing training to ensure clients increase their skills and can become as independent as possible
<li>Identifying potential barriers and working with a team to find ways to overcome them
<li>Learning all requirements of the contract jobs and ensuring clients are properly and thoroughly trained to meet employers’ expectations
<li>Providing coverage for clients, as needed, to meet the daily work requirement
<li>Arriving on site at least ½ hour prior to the first client arriving, unless instructor is responsible for transporting clients to the job site</ul>
<p>Maintains an open line of communication with clients and Opportunity staff so the client is provided with necessary training to become as independent as possible. Duties may include, but are not limited to:</p>
<ul>
<li>Communicating openly with clients with both positive and constructive feedback, while giving options for future independence
<li>Conducting time studies on clients’ work speeds to ensure payment is within wage and hour regulations
<li>Maintaining personal, written, and phone contact with Opportunity Partners staff to monitor the clients’ satisfaction and progress
<li>Completing all required paperwork in a timely, professional, and efficient manner</ul>
<p>Acts as a liaison to ensure the communication channel is maintained between Opportunity Partners, Inc. and the Contact person.  Duties may include, but are not limited to:</p>
<ul>
<li>Developing an open line of communication with the Contact person and soliciting both positive and negative feedback to ensure that the employers’ needs are being met
<li>Informing Employment Services Managers and Service Planners of any changes to the Service Agreement</ul>
<p>Evaluates potential clients through on-the-job evaluations (OJE) so they are given the opportunity to try different jobs in the community.  Duties may include, but are not limited to:</p>
<ul>
<li>Completing OJE daily and turning them in to appropriate personnel on time</ul>
<p>Engages in personal development activities so the OP services offered are up to date.  Duties may include but are not limited to:</p>
<ul>
<li>Participating in required training in accordance with OP policy
<li>Attending workshops, in-service training, and conferences, and sharing new information with OP staff as appropriate
<li>Attending monthly staff meeting</ul>
<p><strong>Qualifications: </strong></p>

<li>Knowledge customarily acquired through a formal education resulting in a high school diploma or equivalent, two years of post high school training and experiences in the rehabilitation field a plus

<p><strong>Purpose</strong><br />
The purpose of this position is to aid in providing consistency of staffing throughout the Learning Options Department and provide instruction to clients to develop functional skills and abilities so that they are accomplish their short and long term goals.</p>
<p><strong>Essential Functions</strong></p>
<p>Provides coverage for Learning Options Instructors so that there is consistency in daily curriculums.  Duties may include, but are not limited to:</p>
<ul>
<li>Teaching classes when instructors are absent
<li>Planning and implementing meaningful curriculum for a variety of classes
<li>Joining an existing group to increase support
<li>Providing 1:1 instruction
<li>Following lists of prioritized coverage/activities on a daily basis
<li>Supervising to insure activities are taking place in a safe manner and that community integration is occurring
<li>Insuring emergency medical information for each client is readily available
<li>Passing required physical and attending training in order to drive the 5310 bus, and assisting with transporting as needed</ul>
<p>Develops and implements methods of instruction so that services outcomes are met. Duties may include, but are not limited to:</p>
<ul>
<li>Completing and implementing daily lesson plans which are geared toward increasing clients’ independence in vocational and daily living skills
<li>Procuring supplies, equipment, and funds within budgetary and agency guidelines</ul>
<p>Participates as a team member so that client services are facilitated and that regulatory and licensing requirements are met.  Duties may include but are not limited to:</p>
<ul>
<li>Documenting client goals and objective progress on a daily basis
<li>Assessing skills and recommending programming once per year for each assigned individual
<li>Preparing for and participating in planning meetings
<li>Providing verbal and written feedback
<li>Reporting attendance on a daily basis
<li>Communicating client related concerns and making recommendations to the service planner on an ongoing basis
<li>Insuring that all client information is relayed using lawful data privacy practices and HIPAA regulations</ul>
<p>Develops and implements curriculum based lesson plans for client goals, keeping in mind individual needs and vulnerabilities.  Duties may include but are not limited to:</p>
<ul>
<li>Creating lesson plans for instruction and methodologies to be used based on the curriculum and client goals, including alternate plans
<li>Assisting management in planning and coordinating program schedules, as needed
<li>Writing class descriptions and curriculum guides, as needed
<li>Procuring supplies, equipment, and funds within budgetary and agency guidelines on an ongoing basis</ul>
<p>Engages in development activities so that services offered are up-to-date.  Duties may include, but are not limited to:</p>
<ul>
<li>Participating in at least 40 hours of training the first two years of employment and 20 hours each year thereafter in seminars/conferences on an ongoing basis
<li>Participates in required training in the Vulnerable Adult Act, Right to Know law, and Blood Borne Pathogen update once per year
<li>Maintaining First Aid and CPR certification
<li>Attending workshops, in-service training, and conferences on an ongoing basis
<li>Participating in required meetings</ul>
<p><strong>Qualifications: </strong></p>
<ul>
<li>Knowledge customarily acquired through formal education resulting in a high school diploma or equivalent. Education or training in the rehabilitation field preferred
<li>Communication skills, both written and oral
<li>The ability to understand and effectively use the P.E.A.R.L. interaction style with program participants, interdisciplinary team members, coworkers, supervisors, and other internal and external customer
<li>Flexibility
<li>Ability to work under pressure and react quickly in emergency situations
<li>Knowledge of development disabilities and similar conditions
<li>Organizational and planning skills
<li>Innovative and creative abilities
<li>Ability to teach, on a repetitive basis, a variety of independent living and leisure skills
<li>Ability to work with a wide variety of people

The  purpose  of  this  position  is  to  provide  oversight  and  consultation  to  the  health  services, procedures and the delivery of medical services relating to the health of all clients through the prevention, recognition and treatment of illness and injury and the promotion of health and safety.</p>
<p><strong>Essential Functions</strong>:</p>
<p>Provides   leadership   and   consultation   services   to   residential   nursing personnel and staff so that standard medical practices are followed, procedures are adhered to, and quality medical services are provided to residential clients. Duties may include, but are not limited to: </p>
<ul>
<li>ensures compliance with regulatory agencies;
<li>ensures procedures relating to medical care are reviewed and satisfy operational needs;
<li>ensures client charts are complete and meet standards; d) provides LPN’s with clinical direction;
<li>reviews, writes and approves all health and medical policies and procedures;
<li>oversees employee training programs;
<li>oversees the psychotropic medication monitoring program.</p>
</ul>
<p>Develops and maintains a system of health records and reports so that there is a central source of health information available. Duties may include, but are not limited to: </p>
<ul>
<li>maintaining sign-off records;
<li>ensure records are filed and maintained according regulatory standards;
<li>participates in a collaborative effort to meet health and safety needs of the clients.</p>
</ul>
<p>Hires,  trains  and  develops  direct  reports  and  oversees the general staff procedures so that programs are adequately staffed with qualified personnel. Duties may include, but are not limited to: </p>
<ul>
<li>delegating;
<li>delineating duties;
<li>conducting orientations, staff meetings;
<li>evaluating direct reports.</p>
</ul>
<p>Assess actual/potential health care needs of assigned clients and houses through observation, interview of client and staff and review of client records so that the clients receive good health care. Duties may include, but are not limited to: </p>
<ul>
<li>providing liaison communication;
<li>coordinating care between health care providers;
<li>developing plan of care for medical/diagnosis/health issues;
<li>completing all health reports;
<li>providing written and educational support;
<li>supervise medication administration and medical treatments.</p>
</ul>
<p>Trains and designs training programs for clients and staff in the health and safety areas of the agency so that good health and safety practices are reinforced.  Duties may include, but are not limited to:</p>
<ul>
<li>provides annual training in specified areas;
<li>makes recommendations on staff training needs;
<li>research areas of possible training topics
<li>create curriculum as necessary.</p>
</ul>
<p>Assumes self-responsibility for professional growth and development so that awareness of current standards for health and safety legislation and legal statutes pertinent to the practice of nursing in occupational health is maintained.  Duties may include, but are not limited to: </p>
<ul>
<li>participating in pertinent organizations;
<li>attending workshops and seminars to maintain licensure.</p>
</ul>
<p><strong>Qualifications</strong>: </p>

<li>Formal education fulfilling RN requirements.

<p>Sets up line or machine work as directed by supervisor so that production is accomplished through good layout techniques and motion economy. Duties may include, but are not limited to:</p>

<li>Staging work to accommodate person skill levels while ensuring quality control</ul>
<p>Suggests and implements goals for people so that they can work on skills that will enable them to reach their highest work performance potential. Duties may include, but are not limited to:</p>

<li>Keeping notes and preparing reports
<li>Reporting behaviors, goal progress, and skill development daily, while providing constructive person contact
