Purpose
The purpose of this position is to provide instruction to clients to develop functional skills and abilities so that they can accomplish their short and long term goals.
Essential Functions
- Develops and implements methods of instruction so that services are provided in a safe, feasible, and financially stable manner. Duties may include but are not limited to:
- Delivering quality classroom/community instruction that follows a lesson plan to deliver the skill acquisition requested by the client and their team
- Writing and implementation of lesson plans
- Following agency health and safety procedures on an ongoing basis
- Maintaining a safe and orderly instruction area on an ongoing basis
- Procuring supplies, equipment, and funds within budgetary and agency guidelines
- Facilitating consistent implementation of written behavior management programs and using de-escalation techniques as necessary
- Upon request for volunteer services, developing one-on-one or small group curriculum and supervising volunteer staff
- Covering breaks, lunches, van duty and other assignments to maintain a staff to client ratio of 1 to 10
- Develops and implements the use of community services and resources so that curriculums can be implemented that offer clients community integration opportunities and meet their needs. Duties may include but are not limited to:
- Identifying, inviting, and securing community resources/guest presenters to assist in the delivery of a message related to the lesson plan
- Identifying, planning, and facilitating a community based lesson plan using local resources to help transfer knowledge learned in the classroom to the community for greater inclusion
- Arranging for outside activities and arranging transportation
- Ensuring supervision, risk management plans, emergency medical information, and photos for each client are readily available
- Supervising to ensure activities are taking place in a safe manner and that integration is occurring
- Following agency health and safety policies and procedures
- Participates as a team member so that client services are facilitated and that regulatory/licensing requirements are met. Duties may include but are not limited to:
- Documenting client progress
- Reporting attendance on a daily basis
- Preparing for and participating in planning meetings
- Communicating remarkable client related concerns and making recommendations to the service planners on an ongoing basis
- Providing verbal and written feedback
- Ensuring all client information is shared using lawful data privacy practices & HIPAA regulations
- Working with team members to ensure client ratio and safety standards are met prior to, during, and after class time
- Develops and implements curriculum based lesson plans for client goals, keeping in mind individual needs and vulnerabilities. Duties may include but are not limited to:
- Creating lesson plans for instruction and methodologies to be used based on the curriculum and client goals, including alternate plans
- Assisting management in planning and coordinating program schedules
- Writing class descriptions and curriculum guides
- Procuring supplies, equipment, and funds within budgetary and agency guidelines on an ongoing basis
- Engages in training development activities so that skills and services offered are up-to-date. Duties may include, but are not limited to:
- Participating in at least 40 hours of training for the first two years of employment and 20 hours each year thereafter
- Participating in required training in the Vulnerable Adult Act, Right to Know law, and Blood Borne Pathogen update once a year
- Maintaining First Aid and CPR certification

Purpose
The purpose of this position is to hire, train, and supervise SET Instructors and SET Floaters, and to oversee operations of assigned SET sites. Acts as a liaison between OP and community employers to ensure we are providing excellent customer service to our community employers. Seeks out new business opportunities with existing community employers and, as possible, generates external referrals for new business from existing community employers and community partners.
Essential Functions
- Supervises the recruitment, hiring, training, and follow-up of SET instructors so that SET sites are adequately covered with qualified staff. Duties may include, but are not limited to:
- Interviewing, hiring, and overseeing the training of new staff in coordination with Human Resources
- Overseeing company and department orientation and job site training
- Interpreting the agency’s policies to employees
- Reviewing staff performance and making salary recommendations for staff within company guidelines
- Overseeing staff meetings conducted by Assistant Manager / SETS to ensure that they meet staff training needs
- Overseeing training opportunities arranged by Assistant Manager / SETS to meet annual training requirements
- Visiting large community job sites at least once a month and smaller community job sites at least once per quarter, documenting in writing your visit and completing a written review of SET paperwork
- Providing regular acknowledgement and feedback for SET Instructors through regular one-to-one meetings, once a week for new staff and at least once a month for existing staff, and as needed based upon employee performance

Coordinates SET services with existing community employers, Opportunity Partners staff, and other necessary parties so that any service issues are minimized and opportunities for existing and new business growth are maximized. Duties may include, but are not limited to:
- Meeting face-to-face with employers at existing large community job sites at least once a month to review community employer satisfaction levels and anticipate and/or respond to employer needs and concerns
- Checking in with employers at existing smaller community job sites at least once a month via telephone or email contact and meeting face-to-face with these employers at least once a quarter to review community employer satisfaction levels and anticipate and/or respond to employer needs and concerns
- Increasing potential for growth at existing community job sites by analyzing all opportunities for expansion/additional services and, with assistance from OP Sales Team and Assistant Director / SETS, approaching community employers with ideas for expansion and/or additional services
- Prospecting for new business opportunities by marketing exceptional quality of SET services to existing community employers and seeking out potential external referrals

Coordinates new SETS with community employers, Opportunity Partners staff, and other necessary parties so that “start up problems” are minimized. Duties may include, but are not limited to:
- Attending meetings in anticipation of new site start – ups and gathering information regarding business / departmental needs
- Assisting to determine viability of new site start – ups, and overseeing activities of Assistant Manager / SETS as they develop potential Client candidate pool with collaboration between SET Department and Service Coordinators and possibly Production
- Hiring and overseeing the training of new staff and supported employment teams
- Maintaining active presence on-site at new SETS throughout training period to ensure seamless service delivery by anticipating and responding to any challenges, issues, or concerns
- Contacting the employer for new site start ups at least: four times per month for first month; one time per month for five months following and; at least one time per quarter for the remainder of the first year

Assures that all SETS are staffed adequately so that site coverage is consistent with community employer needs. Duties may include, but are not limited to:
- Keeping current on all procedures for each assigned SET site
- Communicating with SET floaters and instructors to ensure smooth substitution transitions
- Overseeing site coverage for both staff and clients that has been coordinated by Assistant Manager

Purpose

- Provides supervision to direct care personnel so that services are consistently provided and a positive image of Opportunity Partners prevails. Purpose
The purpose of this position is to plan, manage and design individualized service programs for a client(s), and to provide direction to those clients in conjunction with the total program objectives. The incumbent also manages annual participant assessments and reviews and determines a course of action, and assures compliance with regulatory reporting requirement.

· Manages the provision of rehabilitation services so that clients are able to accomplish or maintain their goals. Duties may include but are not limited to:
o Developing vocational educational action plan

Purpose
The purpose of this position is to instruct, evaluate, and supervise clients to enhance skills that will enable them to meet the expectations of an employer. These tasks are to be completed so that clients increase their skills and their progress is evaluated for possible independent placement. The incumbent will also serve as a working supervisor to ensure that all contracted work is completed.
Essential Functions
- Provides on-the-job instruction so the client is able to meet the expectations of the employer, and works with the client to increase their skills and overcome potential barriers to independent placement. Duties may include, but are not limited to:
- Working with clients to build skills and self-confidence and providing constant feedback, striving for continued individual client progression
- Individualizing training to ensure clients increase their skills and can become as independent as possible
- Identifying potential barriers and working with a team to find ways to overcome them
- Learning all requirements of the contract jobs and ensuring clients are properly and thoroughly trained to meet employers’ expectations
- Providing coverage for clients, as needed, to meet the daily work requirement
- Arriving on site at least ½ hour prior to the first client arriving, unless instructor is responsible for transporting clients to the job site
- Maintains an open line of communication with clients and Opportunity staff so the client is provided with necessary training to become as independent as possible. Duties may include, but are not limited to:
- Communicating openly with clients with both positive and constructive feedback, while giving options for future independence
- Conducting time studies on clients’ work speeds to ensure payment is within wage and hour regulations

Purpose:
The purpose of this position is to provide supervisory support and direct care services to clients and personnel so that expectations of the clients and Opportunity Partners standards are maintained.
Essential Functions:
Assists clients in all areas of daily living so that progress towards independence is reinforced. Duties may include, but are not limited to:
- Providing any necessary assistance to clients to maintain a positive personal image such as grooming, hygiene, clothing coordination, and social skills
- Teaching and documenting clients’ financial management, such as banking, paying bills, shopping, budgeting
- Discussing and encouraging recreational and leisurely activities, as well as acceptable social interactions and community participation, along with conflict resolution skills
- Teaching personal safety skills, such as home safety, emergency disaster procedures, street safety, bicycle safety, vulnerability issues, and using and carrying proper identification
- Teaching clients skills in using metro transit options, when applicable
- Supporting clients’ employment expectations and standards and discussing the importance of responsibility and employment obligations
- Teaching and supporting clients’ personal medical and health care plans
- Delegating responsibility or notifying client employer or day program of any medication/treatment plan changes and appointments at least one week in advance, when applicable

- Scheduling of personnel and posting at least two weeks, but optimally one month, in advance, ensuring all shifts have minimal coverage
- Assisting Program Manager with employee performance appraisals, including maintaining adequate documentation and knowledge to support
- Completing Verbal Performance Notification forms and submitting to Program Manager to document employee performance that does not comply with company standards
- Complying and adhering to Opportunity Partners policies and procedures including, but not limited to, confidentiality practices, completing company accounting responsibilities, following expectations in the Petty Cash Policy, and completing required client financial reports
- Communicating issues of concern that are impacting to the agency, residential programs, and/or clients to the appropriate individuals

Participates in the assessment and individual program planning process with each client, so that outcomes can be obtained. Duties may include, but are not limited to:
- Meeting with each client at least monthly to review program plan
- Maintaining documentation regarding client program plans and medical records to thoroughly and responsibly justify services
- Adhering to specified consumer medical plans to ensure clients receive administration of prescribed treatments and medications
- Maintaining effective communications and positive working relationships with clients’ families

- Assisting in the training of existing staff on changes in client healthcare, programs, vocational issues, etc
- Communicating training gaps that exist to Program Manager and/or LPN and identifying individual and program training needs as necessary
Fulfills requirements of staff development so that clients receive the best possible services. Duties may include, but are not limited to:
- Maintaining certification of training requirements such as: Medication Administration, Vulnerable Adult, CPR, First Aid, and required annual training
Participates in the supervision of the physical environment so that client and staff safety is ensured. Duties may include, but are not limited to:
- Assessing cleanliness, functionality, safety, and security of house and equipment and communicates issues to appropriate individuals
- Performing regular fire and emergency drills as required by state licensing standards and Opportunity Partners’ policy

Purpose
The purpose of this position is to provide coverage to Vocational Evaluation Specialists on an as needed basis and to provide individualized vocational evaluations to people with DD and/or MI and/or TBI at OP program sites and community sites.
Essential Functions
- Evaluation: Evaluates participants in situational assessment settings as requested by referral source so that a rehabilitation plan can be developed that will take into account each participant’s interests, skills and limitations. Designs evaluation plans for individual clients so that needs are identified and evaluation methods are planned. Duties may include, but are not limited to:
- Observing and measuring quality, productivity rate, work behavior, skills and interest of each participant
- Administers tests and assessments as requested by referral source and those tests deemed necessary to make vocational recommendations
- Designing an evaluation plan that takes into account the participant’s history and interests
- Training participants on new jobs on a one-to-one basis
- Participating in the planning process by obtaining necessary information from the Employment Specialist/Case Manager and/or the referral source
- Scheduling and conducting necessary meetings with the interdisciplinary team (IDT) and OP staff
- Developing and/or implementing new tests or programming ideas
- Researching evaluation methods
- Coverage: Provides coverage to the evaluation team as needed. This may include planned and unplanned PTO coverage, and leave of absences. Duties may include, but are not limited to:
- Familiarizes self with daily notes on each participant
- Records and documents progress and writes reports so that all state, federal and CARF requirements are met
- Writing all necessary reports and update memos; such as evaluation summary, Work Adjustment Training reports, transition reports and termination reports
- Completing billing summaries
- Keeping all paperwork and files organized and up-to-date to meet CARF, state and county guidelines
- Marketing: Develops a working relationship with Employers, School Personnel and other referral sources to maintain a fee for service referral base. Duties may include, but are not limited to:
- Maintaining current employer relationships, developing new employer relationships
- Maintaining ongoing referral base
- Determining and meeting participant goals
- Growing the number of community vocational evaluation sites
- Securing authorizations for services provided and completing 30-day status reports for each case
- Attending CTIC or Rehab Services meetings when appropriate
- Vocational Counseling: Provides vocational counseling to participants on case load so that each participant receives an individually developed plan. Duties may include, but are not limited to:
- Counseling one-on-one with participants concerning vocational abilities and those personal issues that impact their performance
- Recommending appropriate services, including Rehab Services, Social Services, HC DD Division support, etc
- Teaching vocational decision making skills
- Providing crisis intervention services as necessary
- Teaching, counseling, and helping to direct the goals of participants in evaluation, Work Adjustment Training, and transition
- Team Work: Participates as a member of the team so that program and client goals are achieved. Duties may include, but are not limited to:
- Participating in scheduled team meetings
- Exchanging resource information, consultation, and technical information with team members
- Supporting team members to achieve program and client goals
- Maintaining a balanced schedule of time in the office as well as in the community performing job duties
- Assisting team members in their job duties when appropriate, when staffing needs dictate, or upon request
- Professional Activities: Engages in developmental activities so that OP services offered are up-to-date. Duties may include, but are not limited to:
- Participating in at least 40 hours within first two years of employment and 20 hours of training thereafter during each year on an ongoing basis
- Attending workshops, in-service training and conferences, and sharing new information with OP staff as deemed appropriate
- Keeping current as to developments and trends in the vocational rehabilitation and evaluation areas
- Participating in in-service meetings
Qualifications:
- A four year degree in social services is required. A Master’s degree in Vocational Evaluation or Vocational Counseling is preferred
- Communication skills, both written and oral
- The ability to understand, effectively use, and train program staff in the use of the P.E.A.R.L. interactional style
- Knowledge of developmental disabilities, MI, brain injury and related conditions
- Organizational skills
- Knowledge of testing and evaluation techniques, including standard and nontraditional (paper/pencil and situational).
- Ability to work with a wide variety of people
- Ability to teach job techniques and duties
- Ability to drive and have access to a dependable car
- Ability to work under pressure and react quickly in emergency situations
- Ability to work in a wide variety of situations, especially packaging and assembly work

Purpose
The purpose of this position is to provide support and training services to the participant in his/her environment and to promote the individual’s development towards near or total independence in the community.
Essential Functions
- Instructs participants in all necessary areas of daily living so that participants experience growth, self-management, and independence to the greatest extent possible. Duties may include, but are not limited to:
- Implementing client ISP as developed by the case manager with positive individualized training style
- Following client interest and team recommendations as listed in outcomes
- Instructing participants in all areas of personal care including health and hygiene, personal possessions, clothes shopping, and apartment maintenance
- Assisting clients with maintaining finances, including medical and financial benefits, and instructing on proper money management
- Assisting client with utilizing community resources per individual need
- Assisting clients with managing their specialized medical needs, following through on recommended medical treatment, and ensuring appointments are completed by due dates
- Teaching personal safety skills and the use of leisure time
- Instructing participants in nutrition, menu planning, grocery shopping and meal preparation
- Assisting client in maintaining current job placement
- Reviewing emergency, household, and community safety information quarterly and addressing vulnerability issues as needed
- Assisting clients in applying for and maintaining eligibility for economic assistance programs, private insurance programs, and social security benefits. Completing forms on time and monitoring resources to meet guidelines
- Leading or co-facilitating one activity per quarter in addition to the three annual program wide activities
- Communicating with appropriate team members regarding significant issues on a timely basis
- Providing on average 290 units or 91% of billable hours per month
- Ensuring that client, family, social worker and other IDT members express consumer satisfaction with services and outcomes via comments and surveys
- Facilitates IDT meetings, reviews outcomes with client and case manager and other team members quarterly, and maintains communication through oral and written reports. Duties may include, but are not limited to:
- Completing annual, semi-annual, and quarterly reports and any other required reports, and submitting by required deadlines
- Chairing IDT meetings and soliciting input from all team members to promote consumer satisfaction
- Developing and reviewing outcomes
- Preparing client for presenting their own review and description of program plan for IDT meetings
- Reviewing Data Practices and Procedures, Consumer Bill of Rights, the social service appeal process and guardianship status with IDT members at 45 day and annual reviews
- Assists new and transitioning participants with moves in the community and assures immediate needs are met. Duties may include, but are not limited to:
- Assisting with making roommate and apartment selection, including lease arrangements and necessary utilities, while ensuring client and team member satisfaction
- Helping client furnish apartment
- Assisting with the actual move and helping client organize their new home
- Assisting client with completing first week’s menu plan, grocery list, and shopping
- Assisting with establishing bank services, completing change of address forms and applying for a new ID within the first month
- Assisting with development of a budget plan and transfers of prescriptions
- Orienting client to local community by beginning development of a reference book, phone directory, and with establishing a routine within first month
- Reviewing emergency procedures within the first 24 hours of program admission
- Reviewing program policies with participants
- Reading the client his/her rights and obtains signatures to be filed within the first 24 hours of program admission
- Maintains records for each client including the relevant data as specified in Consolidated Standards. Duties may include, but are not limited to:
- Documenting exact services provided to client including activities, medical appointments, outcomes, and time involved
- Using ABCD documentation for billing
- Utilizing time efficiently in order to adequately develop, implement and coordinate the clients’ program plans
- Keeping a record of pertinent financial information
- Completing and updating pertinent information on SILS information form by first week of admission, and as need thereafter
- Showing an awareness of caseload needs and contract limits by turning in appropriate monthly schedule the 4th week of the month
- Documenting emergency procedures for all clients every three months on checklist and in summary comments
- Continues ongoing educational training as to render quality services and gain knowledge of new ideas and concepts in the field. Duties may include, but are not limited to:
- Attending staff meetings and in-services
- Attending conferences and seminars
- Reading pertinent and new research periodicals and documenting in training log
- Completing mandatory annual training hours
- Keeping CPR and First Aid Certification up to date
- Other duties may include, but are not limited to:
- Assisting with program emergencies
- Helping with office coverage and answering phones
